1. IMMEDIATELY report the injury to TriageNow 855-638-7984 IF THIS IS A LIFE/LIMB THREATENING EMERGENCY CALL 911 STEP 2 *If a 911 incident, contact your loss control representative immediately

2. TriageNow will gather information from the supervisor and the employee about the injury. Send your injured employee to the occupational medicine clinic that TriageNow assigns to them.

3. Request a copy of the employee’s work status report. Send a copy of the work status report to the Claims Team.

4. Complete injury forms. If appropriate, do this before the employee goes to the clinic. We will send you an email with which documents we need. Not all documents in this packet are required for every injury.
